For those individuals who are 18 years and older please download, print and complete the Adult History Form and bring with you to your first scheduled appointment.
For those individuals who are 17 years or younger please download, print and complete the Child History Form and bring with you to your first schedule appointment. If the child is 13 years or younger the child history from must be completed by a parent. If the individual is 14 years or older he or she may complete this form or it may be completed by a parent.
